Registrations can be sent in two ways:
1.) Mail the forms to the Faith Formation Office at 936 Ninth Street, Green Bay 54304.
2.) Drop the forms off at the Faith Formation Office which is open Monday through Thursday (closed Friday) from 8:00 am until 1:00 pm and additional hours by appointment. Please call 497-7042 ext. 119 for more information.
